The Leaving Care Service

Camden’s Leaving Care service will work with you to support you as you move
towards independence, and our involvement in your life will lessen as you move
closer to this goal and take on more control of your life.

The leaving care service is made up of personal advisors who work with young people aged 18-21 (or up to the age of 25 if you still need support) who have left care.

Once you are 18 and leave care, your social worker will make sure you have an allocated personal advisor. We aim to ensure that you have the same personal advisor throughout your journey to independence who will stay in touch with you to make sure you are getting the support you need.

We recognise that everyone will make this journey towards independence at their own pace, and we will tailor our support to make sure you have the right level of support at all times.

Your personal advisor

Your personal advisor is there to:

  • Provide advice and support
  • Help you to implement and review your pathway plan;
  • Make sure other agencies are providing you with the services you need, for example health or housing;
  • Keep in touch with you to make sure you are okay.
