A Pathway Plan is something you write with your social worker or personal advisor every six months.
It includes information about where you live, what you’re up to and what you want to do. It should also include information about what needs to be done so you get what you need and how this will be done.
Again, you don’t need to do anything. Your social worker or personal advisor will contact you to make a time to meet you to do this plan.
It shouldn’t take longer than it does to sit down with them and have a coffee.
